You Will Be Responsible For:
Coordinates the Performance Improvement and Infection Control internal audit monitoring and completes quarterly reports
Receives initial referral via telephone or fax and inputs patient information in the EMR for SOC and evaluations to be made
Works in conjunction with Clinical Supervisor and Scheduling Coordinator to ensure the SOC visits are made timely in accordance with home care policy
Assists with internal orders and bill hold issues as requested
Completes PI and statistical information as required and requested and submits them to the hospital's PI department
Admissions Coordinator is responsible for the coordination of initial patient information, enabling staff clinicians to have and participate in timely and accurate information for patient care
What You Will Need:
ASN Degree or Diploma in Nursing
Current GA RN license
At least 2 years of nursing experience in home health involving direct patient care preferred
The role of the Registered Nurse (RN) is responsible for the delivery of coordinated interdisciplinary care for a group of patients within a patient care unit (home health) and for the day-to-day management and supervision of the related clinical activities.
The Admissions Coordinator is responsible for the coordination of initial patient information, enabling staff clinicians to have and participate in timely and accurate information for patient care, regardless of payor and patient location. Confirm diagnosis and supply ICD-IO codes appropriately. Work with clinicians in on-going case management/collaboration as appropriate and/or per Clinical Supervisor.
The Performance Improvement Coordinator is responsible for planning, directing, and managing the activities of the performance improvement and related functions of the home health department. Related functions include infection control, staff development, and staff education.
The PI Coordinator is responsible for maintaining an ongoing Performance Improvement Program designed to objectively and systematically monitor, evaluate, and improve the quality and appropriateness of patient care, patient outcomes, and home health services. Assists in the development and oversight of the performance improvement policies and procedures, and departmental objectives.
